Customer Submission History in Simply Forms lets logged-in customers view their previous form submissions directly on your Shopify storefront.
Instead of contacting the merchant to check what they previously submitted, customers can return to the form page and review their own submission history.
Why Show Customers Their Previous Submissions?
Many forms represent an ongoing interaction between a customer and a store rather than a one-time message. For example, customers may want to revisit:

Giving customers access to their submission history provides useful context when they return to the store and creates a more personalized customer experience.
How Does Customer Submission History Work?
Simply Forms associates previous submissions with the logged-in customer and displays the relevant submission history alongside the form. You can configure where the submission history appears:
- Above the form
- Below the form
- To the left of the form
- To the right of the form
- Hidden
You can configure the visibility and placement directly in the Simply Forms app block settings in the Shopify Theme Editor.
What Happens If the Customer Isn’t Logged In?
Submission history is available to logged-in customers so Simply Forms can identify which submissions belong to them.
If a customer isn’t logged in, you can display a sign-in prompt with the form. After signing in, the customer is automatically redirected back to the current page.
This makes it easier for customers to sign in and continue where they left off.
